MedOnSpot Ltd (MOS) is committed to providing quality health care while making it affordable and accessible to patients. Our subscription membership program allows for telehealth enabled Doctors visits with our general practice doctors with in-home visits by our nurses. In an effort to ensure that this service is accessible to those that genuinely need to see a doctor and our service standards are maintained, the following conditions apply:This Agreement supersedes any and all prior written or oral agreements between you and MOS. By signing this Agreement, you agree to the conditions herein.Scheduling Appointments:We encourage you to call in or use our website to set appointments in advance. This will assist us in ensuring you have the shortest waiting time to see a doctor. All appointments must be cancelled by email.System misuseThe following are conditions aimed at reducing our risk of system misuse:

Account may be flagged for several reasons not limited to:

Violence against staff (verbal or otherwise)

Misappropriation of the Medical Service Providers’ time

Indecent behavior of any kind

Persons found making frequent appointments with no apparent physical medical conditions, after being referred to relevant specialists up to and including phycologists and psychiatrists may have their membership revoked.

MOS reserves the right to limit and or refuse access through this subscription service in these instances.

In such an event, your subscription may be terminated, and you will be charged the full cost of all visits engaged for that year’s subscription. The difference will be charged to you if the full costs exceed your subscription cost. Otherwise you will be liable for the full annual subscription amount.

If your subscription is cancelled by MOS for misuse:

You will no longer have access to the program

You will be charged the full cost of all visits for that year’s subscription. The difference will be charged to you if the full costs exceed your subscription cost. Otherwise you will be liable for the full annual subscription amount.

Payment of Annual Fee

The MOS Membership Packages, which is renewable annually, will cover 12 months.

Memberships are non-transferrable

Membership Packages are billed on an annual basis

or through approved monthly deductions by your employer, credit union or other agreed upon means.

Automatic deduction is a payment concession and in no way indicates the total complete value of the membership

Payments are due upon signing up to the program

Automatic payments will be deducted on the 1^st of every month

This is a binding agreement for the duration of the 1 year subscription.

If you withdraw from the service you will be charged the full cost of all visits ($100/virtual consultation & $400/home visit) for that year’s subscription. The difference will be charged to you if the full costs exceed your subscription cost. Otherwise you will be refunded the difference.

Payments more than one week late will be treated as a termination of service and the above will apply.

Access to the membership services will not be available when an account is in arrears.

N.B. This service is intended to provide you with excellent, primary care in a convenient and professional manner. Your prompt care is important to us, However, there may be times when your Physician or another clinical staff member is not immediately available to treat you. This must be considered as this is not intended for emergency medical cases.
